Transforming mental healthcare
The Virgil Healthcare Adult Acute MH D2A Service is a discharge to assess service for patients who are admitted to adult acute mental health units but are stable enough to be stepped down to the community. The service provides provide short-term placement for patients with the support they need to safely transition from hospital to home, while also assessing their longer-term care and support needs in South London and Surrey creating bed capacity for Mental Health Services in London and the South East. Our new and innovate service within the mental health space aiming at improving patient’s outcomes and patient’s experience of the mental health services while working in collaboration with NHS and other Stake Holders

The target market for the D2A service is patients who are admitted to adult acute units but are
Are medically stable and no longer require acute care.
May require some assistance with activities of daily living (ADLs) or instrumental activities of
daily living (IADLs).
May have complex medical or social needs.
Are at risk of readmission to hospital.
