Our services
Our service is a game changer, both on the provision of mental health services and the experience of the services by our patients. We provide seamless experience for patient from Acute services reducing compulsion managing transition back to community reducing relapse rate and improving engagement with the primary services enhancing the experience patients as they are reintegrated into the community which has been a major source of failure leading to relapse and readmission to Acute services. We aim to eradicate instances where patients are kept on the ward due to non-availability of suitable placement or are discharged in a manner which might detrimental to their recovery or negatively impact on their recovery. As a short-term discharge to assess service we work collaboratively with other community services signposting patients to appropriate services in a timely fashion. As we strive for excellence within the mental health space both for experience and service provision the cumulative benefits are numerous.
Assessment
Our dedicated multidisciplinary team collaborates to conduct a thorough assessment that encompasses the intricate interplay of the patient's mental, physical, social, and care needs. This in-depth evaluation extends beyond conventional medical examinations, aiming to comprehensively understand and address the multifaceted dimensions of an individual's well being.
Careplanning
It all begins with you. A personalized care plan will be developed for each patient, in consultation with the patient and their family or caregivers
Coordination of Care
Coordination of care: The D2A team will coordinate the patient's care with other health and social care professionals, such as GPs, district nurses, and social workers.
Support with ADLs and IADLs
The D2A team can provide patients with support with activities such as bathing, dressing, meal preparation, and transportation. The D2A team can help patients to access community resources such as day centres, support groups, and other social activities.
